Peningkatan kualitas secara berkesinambungan adalah hal yang mutlak diperlukan untuk memenangkan persaingan industri dalam dunia manufaktur, kualitas produk juga merupakan hal penting yang harus diperhatikan agar perusahaan tetap survive dalam kompetisi. PT. Kati Kartika Murni (PT. KKM) merupakan salah satu perusahaan yang bergerak di bidang manufaktur paper and packaging product. Meskipun perusahaan ini telah lama berdiri tetapi masih saja terjadi ketidaksesuaian produk yang dihasilkan, yaitu banyaknya produk carton sheet reject yang dihasilkan di Corrugator Section. Metode Six Sigma digunakan untuk meningkatkan kualitas secara dramatik menuju tingkat kegagalan nol (zero defect). Metode Six Sigma memiliki lima langkah-langkah, yaitu Define, Measure, Analyze, Improve, Control (DMAIC). Metode DMAIC akan dilakukan menggunakan beberapa alat bantu peningkatan kualitas, yaitu 5W+1H, p-chart, diagram pareto, fishbone diagram, diagram matrix, dan FMEA (Failure Mode and Effect Analysis). Berdasarkan hasil analisis PT. KKM saat proses produksi carton sheet memiliki kapabilitas kinerja cukup baik yaitu dengan nilai sigma 4,13 dan nilai DPMO 4.332,17 per satu juta produk. Hal ini berarti dari satu juta kesempatan produk carton sheet terdapat kemungkinan 4.332,17 carton sheet yang gagal produksi akibat kerusakan pada carton sheet. Dengan menggunakan metode FMEA maka dapat diperoleh dan dipilih 3 faktor penyebab cacat yang memiliki nilai RPN terbesar dari setiap cacat dominan, sehingga dapat diusulkan rekomendasi aksi yang dapat dilakukan untuk mengeliminasi faktor dominan penyebab cacat overlap dan krepek pada Carton Sheet tersebut.

Continuous quality improvement is absolutely necessary to win the industry competition in manufacturing, product quality is also an important thing to be aware of in order to keep the company survive in the competition.PT. Kati Kartika Murni (PT. KKM) is one of the companies engaged in manufacturing paper and packaging product. Although the company has long been established but still there is a product mismatch that is produced, that is the number of carton sheet reject products produced in the Corrugator Section. The Six Sigma method is used to dramatically improve quality towards zero defect. The Six Sigma method has five steps: Define, Measure, Analyze, Improve, Control (DMAIC). The DMAIC method will be carried out using several quality enhancement tools, namely 5W + 1H, P-chart, Pareto diagram, Fishbone diagram, Matrix diagram, and FMEA (Failure Mode and Effect Analysis). Based on the results of the analysis of PT. KKM when the production process of carton sheet has a good performance capability that is the value of Sigma 4.13 and DPMO value 4,332.17 per one million products. This means that one million carton sheet product opportunity There is a possibility of 4,332.17 carton sheet that failed production due to damage in carton sheet. By using the FMEA method can be obtained and selected 3 factors cause defects that have the largest RPN value of any dominant defect, so that it can be proposed action recommendations that can be done to eliminate the dominant factors causing overlap and crepek defects in the Carton Sheet.
