Sistem rekam medis merupakan beberapa rangkaian dari sistem yang ada dalam proses penyelenggaraan rekam medis. proses ini dimulai sejak pasien datang ditempat pendaftaran dan berakhir hingga pasien pulang/keluar dari institusi pelayanan kesehatan. awal dari sistem rekam medis ini ditempat pendaftaran pasien rawat jalan, baik di poliklinik maupin diunit/instalasi gawat darurat yang meliputi wawancara terkait data identifikasi pasien dari dan registrasi pasien. bila kita melihat proses penyelenggaraan rekam medis secara keseluruhannya, maka pada modul 2A ini khusus membahas registrasi dan pengindeksan lainnya pada modul tersendiri.
