Laporan operasi merupakan catatan mengenai prosedur pembedahan. Pengisian data laporan operasi di Rumah Sakit Kanker Dharmais diidentifikasi kelengkapannya antara lain surat pengantar permintaan dirawat, taksiran biaya operasi berencana, pendaftaran pasien operasi di Instalasi Bedah Sentral, persetujuan tindakan kedokteran bedah, penandaan lokasi operasi ( Site marking ), assesmen pra operasi, hasil laboratorium, Radiologi, konsultasi Jantung, konsultasi Anastesi, konsultasi lain. Tujuan penelitian untuk menidentifikasi kelengkapan dan faktor ketidaklengkapan berkas pasien rawat inap rencana operasi dengan menggunakan metode deskriptif kuantitatif dengan observasi dan wawancara. Hasil penelitian observasi proses pendaftaran pasien rawat inap rencana operasi sesuai dengan SPO No. BRM/SPO.01. REV 04 Tahun 2019 tentang prosedur pendaftaran pasien rawat inap rencana operasi. Dari sampel 90 rekam medis ditemukan 79 rekam medis (87,8 %) lengkap dan 11 rekam medis (12,2% ) tidak lengkap pada formulir konsultasi jantung dan anastesi. Hal ini disebabkan karena pasien baru melakukan konsultasi saat di ruangan perawatan. Ketidaklengkapan menyebabkan keterlambatan proses administrasi pendaftaran rawat inap karena membutuhkan waktu untuk konfirmasi terlebih dahulu. Saran agar prosedur pasien masuk rawat inap rencana operasi dilakukan sesuai SPO sebelum masuk rawat inap
