PT. XYZ merupakan perusahaan yang bergerak pada bidang industry polimer/resin ramah lingkungan. PT. XYZ memiliki lima lini produksi dengan proses produksi yakni mixing dan mesin ekstrusi untuk menghasilkan produk. Dari lima lini produksi tersebut, mesin 3 sering mengalami kerusakan yang mengakibatkan mesin berhenti beroperasi. Tujuan dari penelitian ini yaitu menghitung performance mesin dengan metode Overall Equipment Effectiveness (OEE) dan menentukan faktor- faktor yang menyebabkan kegagalan pada mesin TE 3 dengan metode Failure Mode and Effect Analysis (FMEA). Dari hasil perhitungan pada mesin 3 dengan rentang waktu Januari 2019-Desember 2019 rata-rata nilai OEE sebesar 70,49%. Setelah itu dilakukan analisis menggunakan Failure Mode and Effect Analysis (FMEA). Hasil nilai RPN tertinggi yakni sebesar 448 pada proses heater (pemanasan) pada mesin yang menjadi komponen kritis dalam penelitian ini.

XYZ Inc. is one of manufacturing companies that produce eco friendly polymer/resin. Five production machines were used to do some production process such as mixing and extracting. Production machine number three often suffered trouble that caused production stop. the purpose of this research is to calculate performace of production machine number three using Overall Equipment Effectiveness (OEE) and analyze the elements that caused failure of production machine number three using Failure Mode and Effect Analysis (FMEA). The data that used in this research was January to December 2019. The result showed that average OEE value is 70,49%, the highest RPN value is 448 on heating process and critical component of production machine number three was heater
