PENGARUH PENDAMPINGAN KADER KESEHATAN TERHADAP PENINGKATAN PENGETAHUAN, SIKAP, DAN PRAKTEK PERAWATAN METODE KANGURU (PMK) PADA IBU DENGAN BAYI BERAT LAHIR RENDAH (BBLR) DI JAKARTA UTARA

Perawatan BBLR yang optimal dan keberlanjutan dengan Perawatan Metode Kanguru (PMK) di rumah sangat diperlukan untuk menjaga agar suhu bayi tetap stabil dan menghindari terjadinya gangguan kesehatan pada bayi khususnya neonatus. Namun perbedaan kondisi antara rumah sakit dan rumah akan berpengaruh pada praktek PMK ibu di rumah, dimana praktek PMK di rumah tidak seberhasil praktek PMK di rumah sakit. Penelitian ini bertujuan untuk menganalisis pengaruh pendampingan kader kesehatan terhadap pengetahuan, sikap, dan praktek PMK pada ibu dengan BBLR. Penelitian dilakukan terhadap 100 ibu yang mempunyai BBLR paska perawatan RSUD Koja, Jakarta Utara dengan pendekatan kuasi eksperimen (50 ibu kelompok intervensi dan 50 ibu kelompok kontrol). Teknik pengambilan sampel dilakukan secara consecutive sampling. Di kelompok intervensi ibu BBLR mendapatkan booklet dan memperoleh pendampingan PMK 3 kali oleh kader kesehatan berupa kunjungan rumah yang diberikan pada 3 hari setelah BBLR keluar dari RS, minggu ke-2, dan ke-3. Sedangkan di kelompok kontrol, ibu BBLR hanya mendapatkan booklet dari kader kesehatan pada saat kunjungan akhir di rumah. Pada kedua kelompok dilakukan pengukuran sebanyak 4x yaitu sebelum pendampingan 1, setelah pendampingan 1, setelah pendampingan 2, dan setelah pendampingan 3. Pengumpulan data kualitatif dilakukan untuk melengkapi informasi hasil kuantitatif yang didapatkan dalam penelitian. Analisis bivariat dilakukan dengan independent t-test dan paired t-test, serta analisis multivariat dilakukan dengan General Linear Model-Repeated Measure untuk menganalisis pengaruh pendampingan kader kesehatan terhadap pengetahuan, sikap, dan praktek PMK ibu dengan mempertimbangkan faktor confounding. Hasil penelitian membuktikan bahwa pendampingan PMK oleh kader kesehatan dapat meningkatkan pengetahuan PMK ibu sebesar 14,46% (pengukuran 2), 3,16% (pengukuran 3), dan 1,70% (pengukuran 4); sikap PMK ibu sebesar 10,83% (pengukuran 2), 16,92% (pengukuran 3), dan 2,44% (pengukuran 4); serta praktek PMK ibu sebesar 39,07% (pengukuran 2) dan 7,38% (pengukuran 3). Disarankan adanya pelibatan kader kesehatan dalam pendampingan dan pemantauan PMK di rumah. PMK di komunitas merupakan salah satu strategi untuk menurunkan angka kematian neonatal.

The optimal low birth weight (LBW) infants care and the continuity of Kangaroo Mother Care (KMC) at home are needed to maintain the baby s temperature stable and to avoid the health problems among infants, especially neonates. Practices of KMC at home significantly influenced by hospital care for KMC, and there is a noted difference between hospital care and home care. Often the practice of KMC at home is not as successful as the practice of KMC in hospitals. This study aims to analyse the effect of community health workers coaching towards the development of knowledge, attitude, and practices of KMC among mothers with LBW infants. The study was conducted on 100 LBW infants mothers post-discharge from Koja District Hospital, North Jakarta with a quasi-experimental approach (50 mothers in the intervention group and 50 mothers in the control group). Sampling techniques are consecutive sampling. In the intervention group, LBW infants mothers received coaching of KMC provided by community health workers in the form of home visits 3 days after LBW infants discharge from hospital, 2 nd week, and 3 rd week. Control and intervention group groups were measured 4 times before first coaching, after 1 ix st coaching, after 2 coaching, dan after 3 rd coaching. Qualitative data collection was conducted to complete the information of quantitative results. Analysis was carried out using independent t-test, paired t-test, and general linear model-repeated measure to analyze the effect of community health workers coaching towards the increase of knowledge, attitude, and practice of KMC by considering confounding factors. The research proves that KMC coaching by community health workers are able to improve KMC knowledge by 14,46% (2 nd measurement), 3,16% (3 rd measurements), and 1,70% (4 th measurements); KMC attitude by 10,83% (2 nd measurement), 16,92% (3 rd measurements), and 2,44% (4 th measurement); and KMC practice by 39,07% (2 nd nd measurement) and 7,38% (3 measurements). Recommends community health workers to involve in coaching and monitoring KMC at home. Community KMC is one of reliable sources of strategy for decreasing Neonatal Mortality Rate.
