CD40 merupakan salah satu anggota TNFR yang secara umum diekspresikan pada permukaan sel B. Aktivitasnya dipengaruhi oleh CD40L (CD154) pada sel T CD4+ yang teraktivasi. Interaksi CD40-CD154 berperan dalam regulasi imun, terutama memediasi respon sel B yang dipengaruhi sel T. Interaksi CD40-CD154 yang abnormal dapat mengakibatkan terjadinya GD, dengan mempengaruhi aktivitas sel tiroid. Polimorfisme gen CD40 telah dilaporkan dapat meningkatkan proses translasi dan ekspresi protein CD40 yang berpengaruh terhadap patogenesis GD.
